Resolving a Treatment Dispute
Submit a treatment dispute request by completing the Treatment Dispute Form below. DentiCare will notify your consulting practice and provide updates along the way. Please make sure you provide the following information to assist your request for a treatment dispute:
- Provide a written statement to explain the reason for your treatment dispute
- Upload your original treatment plan and contract agreement
What happens after you apply?
You will receive confirmation of your treatment dispute request and your consulting practice will be notified. Following a discussion with your consulting practice to determine the outcome, you will either receive notice of an approved treatment dispute or a rejection which will include the reasons for the decision; or you may be requested to supply further information for the practice to finalise a decision.
