Once you’ve agreed upon your payment plan with your dental or orthodontic practice you’ll receive an SMS notifying you that your proposed payment plan is ready. Follow the link to review your payment plan details.

Review your proposed payment plan information. You’ll need to enter your address and email to proceed.

Email Verification

Enter and verify your email address using a 6-digit code.

Step 3

Compliance Acknowledgement

Answer eligibility questions to determine if you qualify for a DentiCare Plus or DentiCare Connect Payment Plan.

Step 4

Identity Verification

Enter a valid ID to verify your identity.

If you don’t provide a valid ID, we may contact you and progress with manual identification.

Step 5

Payment Reference

Select your preferred payment method.

Step 6

Payment Plan Summary

You will receive a completed proposal including plan duration, amount, frequency and start date. Agree to the terms and conditions before proceeding.

Your plan has been submitted

Payment Plan Confirmation

If you pass eligibility requirements, you will receive email confirmation that a DentiCare Plus Payment Plan has been established.

If you haven’t provided valid identification or you don’t meet compliance eligibility, you will receive a pending confirmation notification.
